I confirm that the treatment is for a person aged 18 to 60 who has previously vaginal thrush diagnosed by a doctor or healthcare professional, and that their current symptoms are consistent with a previous episode of thrush. I confirm that the intended user does not have liver disease, kidney disease, heart problems including heart rhythm problems, diabetes, any other long-term medical condition, a weakened immune system, or a known intolerance to galactose, Lapp lactase deficiency or glucose-galactose malabsorption. I confirm that the intended user is not pregnant, breastfeeding, or trying to become pregnant. I confirm that the intended user is not allergic or hypersensitive to fluconazole, other antifungal medicines, thrush treatments, or any of the ingredients in the medicine. I confirm that the intended user has not had more than two episodes of thrush in the last six months. I confirm that the intended user has no history of a sexually transmitted infection and has not been exposed to a partner who may have a sexually transmitted infection. I confirm that there is no abnormal or irregular vaginal bleeding, blood-stained discharge, vulval or vaginal sores, ulcers or blisters, lower abdominal pain, pain when passing urine, fever, chills, nausea, vomiting, diarrhoea, or foul-smelling vaginal discharge. I confirm that the intended user is not taking terfenadine, astemizole, cisapride, pimozide, quinidine, erythromycin, or any other medicine that may interact with fluconazole, other than the contraceptive pill. I understand that one treatment course is a single 150mg fluconazole capsule and that no more than one capsule should be taken within a seven-day period. I agree that the intended user will read the patient information leaflet before taking the medicine, including the warnings, side effects and instructions for use. I confirm that the information provided is accurate and complete, and I understand that treatment may be refused if the pharmacist decides it is not clinically appropriate.
